Chronic diseases telemonitoring
Description and scope
The project consists in monitoring a group of chronic patients with multiple pathologies through the use of telemedicine devices in order to ascertain if hospital readmissions are reduced when prescribed treatments are continually monitored.
The project aims to improve the quality of life of patients by avoiding readmissions and visits to healthcare centres and will also seek to quantify the economic savings for the Andalusian Public Health System when its chronic patients are continually monitored and cared for.
“Chronic diseases tele-monitoring" is in an initial pilot phase. If future results are satisfactory, however, the possibility of extending the project to all chronic patients in Andalusia will be considered..
